For the convenience of our campers, the "old" registration form below may be printed and sent via USPS. Alternatively, the "new" form on the ACFO web site may be filled-in and submitted electronically, with registration payment sent via USPS. In either case, the registration payment must be received by May 15 in order to avoid late fees. Registration fees are non-refundable after May 15 and cannot be applied to room and board.
Before filling out the Registration Form, please acquaint yourself with the information found on the Lodging and Rates pages of this site. Fill out the pdf form in Acrobat Reader and print, or print the form and legibly fill in all the requested information. Send the form with payment attached to the Alabama CFO Registrar: Vickey Dunn, P.O. Box 530463, Birmingham, AL 35253.

Medical Release Form for Minors
The insurer for all of the CFO's requires that a "Medical Release" be on file for all minors (under age 18) attending camp. This form (2 pages) must be submitted, with proper authorization signatures, at the time of registration.
